A New Day in Consultation Services have arrived.  Welcome to the New, Improved AC Medical Group.  

Here's how to use our service......Just follow these simple instructions:

1.  Create an account with us by entering your information after clicking the "Request Consultation" tab located above.  Be sure to enter your credit card information and complete the medical questionnaire in its entirety.

2.  Fax us your medical records including your diagnosis, treatment plan....i.e. your previously-prescribed medication (mandatory, we need proof you've been prescribed this medication before), and you must include a physical performed within the last 12 months.  Your documentation must legibly list your Dr'(s) contact information. (IMPORTANT: Your records must include your X-Ray or MRI report if applicable to your condition and a Physical Exam within the Previous 12 mos.. If you have the MRI report, be sure to send it).  Fax these items to our HIPAA-compliant encrypted fax server at 877-245-9969, along with a legible copy of your Government-Issued Photo ID. 

(To ensure you receive your discount, be sure to enter your applicable Discount Code in the space provided during registration.  If you forget, just email us and we'll get you taken care of).

3.  Indicate the best time for the Physician to contact you.  Please allow the Dr a 2hr time slot (still better than Comcast! ;-), as consultations may overlap, as the Practitioner gives each Patient the time they deserve during their consultation and do not under any circumstances rush our Patients.  This is not a push-through service.

4.  You will be contacted for a consultation within 24 hours of receipt of your acceptable medical records (you must include a recent physical stating your diagnosis and medication prescribed as well as an MRI or Xray in most cases, please see above for detailed records requirements) and a legible copy of your Government-issued ID, front and back.
